RN, Referral Coordinator
About the role
Provide care to client employees in our Health Center located in Mt. Sterling, IL. The scheduled hours Monday and Thursday 5:00–7:00 PM are required, with the remaining 36 hours worked flexibly within clinic hours.
Owns the referral process from start to finish, coordinating authorizations, documentation, specialist selection, scheduling, member communication, and follow-up to ensure timely access to care. Provides concierge-level support throughout the referral journey by proactively addressing barriers, coordinating with specialty offices, communicating next steps, and tracking referrals through completion. Closes the loop on referrals by confirming appointments, obtaining specialist records and recommendations, updating the care team, and coordinating follow-up to ensure continuity of care.
Responsibilities
- Assesses patient care needs, conducts nursing evaluations, and provides treatment within scope of practice
- Collaborates with physicians and other healthcare practitioners for further medical evaluation and care as needed
- Performs and documents medical tests, screenings, and treatments as defined in site contract (EKG, audiometry, spirometry, vision)
- Administers medications and emergency care within legal scope of practice
- Maintains medical equipment, ensures a clean and safe environment, and follows infection control procedures
- Maintains patient health records to ensure accurate and up-to-date records
- Educates patients on health promotion and participates in follow-up care
- Performs other duties as assigned
Requirements
- Certificate from an accredited Nursing School or associate degree (A.S.) is required; Bachelor's degree (BSN) from four-year college or university is preferred
- Currently licensed Registered Nurse in state of practice required
- Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
- Certification in Occupational Health Nursing (COHN/COHN-S), Emergency Nursing, or Case Management preferred and may be required for some sites
- At least three years of experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic

Pay
Premise provides its reasonable and genuinely expected range of compensation for this job of $30.00 - $45.00 per hour. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
