Referral Coordinator - RN

Role Overview This position involves processing referrals accurately and efficiently, ensuring compliance with agency policies and regulations, and serving as a resource liaison across product lines to promote customer satisfaction. The role includes screening referrals to verify patient eligibility, obtaining necessary insurance pre-authorizations, and assisting with billing and medical policy interpretation. Additionally, the candidate will track current clients' hospitalization or placement status and coordinate discharge planning while educating internal and external stakeholders about home care services. Qualifications & Skills Minimum of a Nursing Diploma or Associate's degree; RN license required.2-5 years of experience; additional preferred qualifications include a Bachelor's degree in Nursing, Social Work, or related field, with 5-10 years of experience.Excellent communication skills, attention to detail, and ability to coordinate across multiple locations. Key Duties: Support IL team needs, support hospital locations, and facilitate home care referrals and discharge processes. The role primarily involves working at a hospital-based referral center, supporting hospice and palliative care services in a fast-paced environment.
