Responsibilities
- Performing client assessments to ensure effective and appropriate care
- Managing case coordination, supervision, and implementation of nursing services
- Responding to referral information and client inquiries
- Directing and initiating appropriate actions in-home care situations
- Adhering to agency policies, procedures, and community standards
- Completing additional duties as assigned by leadership
Qualifications and Skills
- Graduate of an accredited nursing program; Baccalaureate preferred
- Current RN license in Colorado
- At least 2 years of nursing experience, including long-term care, dementia, or acute care; home care experience preferred
- CPR certification
