About the role

You’re the life preserver for countless patients because, as a Patient Navigator Nurse, you’re the hub of your patients’ multidisciplinary team advocating, interpreting, coordinating, educating, supporting, and counseling along their care journey. As a clinical resource with expertise in specialty care management, you are that critical connection and liaison for a patient population recovering from strokes, cancer, and long-term illnesses. Responsibilities

  • Coordinate and communicate written and verbal information between the patient, family, physician, physician staff, and other team members.
  • Coordinate and establish the continuity of patient care with internal and external departments and agencies.
  • Aid patients in understanding their diagnosis, treatment options, and the resources available.
  • Review medical documentation to assist physicians in obtaining complete and accurate documentation of their medical treatment plan.
  • Act as a critical link between patients and all other care providers, serving as a liaison and facilitator for consults and support services including social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.
  • Collect and track data, support strategic planning processes, and evaluate patient care outcomes.
  • Provide nursing care and guidance from screening to survivorship, assisting patients and their family members in search of solutions, treatment-related problems, and education to facilitate seamless care.
  • Support patients with difficult decisions and assist in ongoing review and updating of policies related to service provided.
  • Participate in community educational events and staff education as a resource.

Requirements

  • Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
  • American Heart Association BLS certification.
  • Minimum of 3 years of professional nursing experience in specialty areas.

Pay

The salary range for this position is $XX/X based on experience and qualifications. Exact pay will be determined during the interview process.

This position is full-time and works days.
