RN Patient Logistics - FT at St Peters Hospital

About the role

RN Patient Logistics full-time days on a rotating shift at St. Peter’s Hospital in Albany, NY. The C3 Patient Logistics team is willing to train the right candidate who meets the minimum requirements and demonstrates the desire and ability to learn and grow into this opportunity to advance their nursing career.

Responsibilities

• Receive calls throughout the organization and externally to facilitate patient placement into the facility
• Triage calls and problem-solve using critical-thinking skills to de-escalate situations while maintaining customer service and patient satisfaction
• Work closely with the Case Management team to ensure timely and accurate patient processing and satisfaction
• Travel to Samaritan and Albany Memorial Hospitals as needed, primarily during orientation

Requirements

• Current and valid New York State RN license
• Bachelor of Science in Nursing, or willingness to pursue a defined plan for matriculation demonstrated by ongoing coursework each semester
• 3 years of experience in an acute care setting (critical care preferred)
• Ability to orient Monday–Friday on a flexible day schedule

Preferred Requirements

• 1+ year of experience in case management or utilization management
• Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM)
• Discharge planning and community resource knowledge
• Maintain the highest level of customer service while completing day-to-day duties and responsibilities
• Critical-thinking skills to problem-solve
• Excellent time management and organizational skills
• High level of efficiency to communicate in a fast-paced work environment across diverse teams and backgrounds
• Engage within a team setting to help promote a strong culture
• Passion to learn, grow, and develop in the nursing field

Pay

$38.00–$50.18 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
