Job Summary
The Patient Logistics RN supervises the transfer center activities for Dignity Health Facilities. The position functions as the supervisor during their assigned shift to support the department and staff to carry out the patient logistics program responsibilities.
Responsibilities
- Supervises transfer center activities for Dignity Health Facilities
- Manages transfer calls, supports staff, and ensures efficient patient logistics
- Conducts quality assurance reviews and compiles data for compliance
- Gathers data for transfers, determines facility designation, and prioritizes based on urgency
- Communicates effectively with internal/external sources and hospital staff for patient flow
- Maintains accurate documentation and participates in professional development and safety initiatives
Requirements
- Current RN license
- Proven proficiency with all Microsoft Office applications
- Familiarity with TeleTracking, Cerner and Midas
- Excellent verbal and electronic communication stills, including professional telephone and email etiquette
- RN with 3-years of clinical experience, one to include acute hospital
- Ability to communicate effectively with a variety of customers including patients, providers, hospital and office staff, health plans, medical groups, internal departments, community resources, and peers
- Ability to work independently and apply critical thinking skills toward making appropriate decisions, as well as in a team environment
- Able to multi-task and prioritize
- Detail oriented
- Excellent organizational and communication skills and ability to meet timeframes
- Ability to learn quickly and retain complex information
- Travel as needed
- Able to lift a minimum of 20 pounds
Preferred Qualifications
- Progressive responsibilities leading up to or including supervisory role
- Experience in Utilization Management, Case Management or Care Coordination, Managed Care
