Description

RN Patient Logistics 8hr FT St. Peter's Hospital in Albany, NY is looking for an experienced RN to work as a Registered Professional Nurse as part of the C3 Patient Logistics team full time days.

Responsibilities

• Call throughout our organization and externally to get patient into the facility
• Triage calls
• Problem solve and utilize critical thinking skills to deescalate and maintain customer service and patient satisfaction
• Work closely with the Case Management team to insure the patients are processed timely while maintaining accuracy and patient satisfaction
• Travel to Samaritan and Albany Memorial Hospitals as needed primarily during orientation

Requirements

• Current and valid NYS RN license
• Bachelor of Science in nursing, or willingness to pursue defined plan for matriculation in program demonstrated by ongoing course work each semester
• 3 years of experience in acute setting (critical care preferred)
• Willingness to orient during the week Monday – Friday on a flexible day schedule

Qualifications

• 1+ year of experience in case management or utilization management
• Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM)
• Discharge planning and community resource knowledge
• Critical thinking skills to problem solve
• Excellent time management and organizational skills
• High level of efficiency to communicate in a fast pace work environment across people and teams of different backgrounds and demographics
• Engage within a team setting to help promote a strong culture
• Passion to learn, grow and develop in the nursing field

Pay

Pay Range: $38.00-50.18hr Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
