Position Highlights

Position: RN Patient Care Navigator – Stroke Program
Location: Evanston Hospital
Part Time: 24 hours/week
Hours: 2 x 12-hour shifts – 7:00AM-7:00PM during the week

What You Will Do

The Stroke navigator will render professional nursing care to assist neurologist, physicians, and APPs in coordinate the care of patients throughout the stoke continuum with primary focus on acute stroke alerts, supporting acute interventions, and collaborating with multidisciplinary team to ensure timely evidence-based care. Ability to effectively function during emergent situations and manage multiple priorities. Employs critical thinking skills, confident neurological assessments, adheres to appropriate treatment protocols for specific emergencies, effectively communicates plan, assists with transport to emergency testing and transfer to higher level of care as indicated. Synergizes with stroke coordinator and neurosciences team to ensure care delivery meets comprehensive stroke center standards including helping maintain stroke program data. Provides stroke education to patients, families, and healthcare staff. Ultimately, this role serves as a clinical resource for stoke protocols, quality improvement initiatives, and regulatory compliance.

What You Will Need

  • Bachelor's Degree in Nursing
  • Minimum three years of RN acute care experience in the treatment of stroke patients
  • Current State of Illinois Registered Nurse licensure
  • Current CPR Certification (BLS) issued by American Heart Association or American Red Cross
  • Current ACLS Certification issued by American Heart Association, or obtain within 3 months of start date
  • Current National Institute of Health Stroke Scale (NIHSS) Certification

Education, Experience And/or Skills Preferred

  • Minimum 3-5 years of RN critical care experience in an acute care setting (emergency or critical care)
  • 5 years experience inpatient stroke care setting
  • Experience delivering care to patients diagnosed with acute stroke
  • Strong communication skills
