About the role
Provides professional team-based care to patients. Assists in the continuum of care and in educating patients on their patient-centered medical home.
Responsibilities
- Performs general nursing care to patients.
- Screens patients for appropriate information.
- Administers prescribed medications, tests, and treatments in accordance with nursing standards.
- Utilizes nursing knowledge and skills in the safe implementation of basic preventative and therapeutic nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
- Assists with the preparation of equipment and aids physician during treatment, examination, and testing of patients and documents accurately in the patient's chart.
- Performs delegated independent nursing functions using established procedures, policies, guidelines, and standards.
- Responds to and refers incoming telephone calls.
- Instructs patient and family regarding medications and treatment instructions, within scope of practice.
- Responsible for preparation of exam rooms including re-stocking of treatment areas.
- Works collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback.
- Participates in professional development activities and maintains professional affiliations.
- Performs related work as required.

Requirements
RN
- Graduation from an accredited school of nursing
- Possession of a State Registered Nurse License
- CPR Certification
- One year of professional nursing experience in a clinic setting preferred
- Word processing and computer experience
LPN
- Graduation from an accredited school of practical nursing
- Valid state license to practice practical nursing
- CPR Certification
- Word processing and computer experience preferred
