We provide skilled nursing care to people recently discharged from the hospital, needing therapy, and to those needing long-term care.

Responsibilities
- Assist in implementing and revising care plans
- Monitor health conditions and supplement direct care needs
- Respond to emergencies or changes in resident status
- Observe care assignments and manage coverage needs
- Evaluate staff performance, educate, and lead staff
- Communicate changes in resident conditions to families and providers
- Collaborate with the interdisciplinary team
- Review and investigate incidents
- Transcribe orders and audit medical charges
- Maintain record compliance for accuracy
- Explain health status, changes, and care expectations to families, residents, and staff
- Guide and direct care skills with intervention as needed
Requirements
- Licensed to work within the State of Connecticut as an RN (Registered Nurse)
- Able to pass background check, drug screen, professional references check, and physical evaluation
- Strong leadership and clinical competency
- Interest in growth and career development
