RN - Nurse Coordinator

About the role

Provides advanced nursing support within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management, supporting primary clinical functions for patients, families, and providers.

Responsibilities

• Assess healthcare, educational, and psychosocial needs; develop individualized care plans.
• Coordinate diagnostic tests and facilitate communication among patients and healthcare team.
• Educate patients and families on disease management and care coordination using age-appropriate strategies.
• Implement clinical interventions based on risk and evidence-based guidelines, collaborating with providers to adjust therapies and ensure adherence.
• Maintain documentation, manage patient interactions, and assist with triage and clinical workflows.
• Administer medications, vaccines, and IVs within scope of practice, and prepare laboratory specimens.
• Participate in professional development and post-discharge care coordination to improve patient outcomes.

Qualifications and Skills

• Minimum of 2 years RN experience.
• Graduate from Specialty Training Program-Nursing (required); Bachelor's preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work collaboratively within a team in a clinical setting.
• Commitment to quality improvement, patient-centered care, and continuous learning in a fast-paced clinical environment.
