RN - Nurse Coordinator

About the role

This position involves serving as an expanded nursing team member to promote preventative care, conduct screenings, and coordinate disease management services. The nurse provides primary clinical support to providers, patients, and families, assisting in diagnosis and treatment planning.

Responsibilities

• Assess healthcare, educational, and psychosocial needs, involving patients and families in establishing treatment goals.
• Facilitate communication among healthcare team members to address barriers and identify intervention options.
• Educate patients and families on disease management, case management programs, and clinical guidelines.
• Implement clinical interventions based on risk assessments and evidence-based practices.
• Coordinate laboratory and diagnostic testing, ensuring accurate documentation and follow-up.
• Assist with patient triage, manage in-basket messaging, and develop individualized care plans.
• Administer medications, vaccines, and perform IV insertions and monitoring within scope of practice.
• Maintain accurate records in electronic systems and participate in ongoing professional development.
• Support post-discharge care coordination and contribute to program design and evaluation.

Qualifications & Skills

• Minimum of 2 years RN experience.
• Graduate from a Specialty Training Program in Nursing; Bachelor's Degree preferred.
• Strong clinical, communication, and patient education skills.
• Ability to collaborate effectively within interdisciplinary teams.
• Proficiency in clinical documentation and diagnostic coordination.
