RN Navigator - Coordinated Treatment Center

About the role

RN Navigators at Coordinated Treatment Center collaborate closely with multidisciplinary teams across a diverse range of patient populations and diagnoses to coordinate comprehensive, patient-centered care.

Responsibilities

• Rooming patients, obtaining vitals, weights, and other clinical measures
• Supporting team members to ensure patients are seen by the appropriate disciplines
• Monitoring patient progress to confirm orders and education are completed
• Addressing ongoing patient needs through phone, MyChart, and other follow-up communication methods
• Mon-Fri schedule
• $15,000 sign-on bonus

Qualifications

• Bachelor's degree in Nursing strongly preferred
• Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics)
• Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position
• Graduate from an accredited nursing program
• Minimum of two years’ clinical experience preferred
• Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
• Obtains and subsequently maintains required department specific competencies and certifications

$34.00 - $51.00 per hour

Schedule

Full time, 40.00 weekly hours, 8 Hours - Day Shifts
