General Duties
- Captures written and verbal communication between patient, family, physician, and team members.
- Coordinates treatment plans with internal and external departments and agencies.
- Aids patients in understanding their diagnosis, treatment options, and available resources.
- Reviews medical documentation to assist physicians in accurate documentation.
- Serves as a bridge between patients and all care providers.
- Facilitates appointments for consults and support services.
- Follows patients through the care continuum, including inpatient admissions.
- Collaborates with inpatient care management resources.
- Collects and tracks data for service line personnel.
- Supports the patient during difficult decision-making periods.
- Functions in an organized and time-conscious manner.
- May initiate and update policies related to service provided.
- Serves as a resource for community and staff education events.
Education Qualifications
Preferred: Bachelor’s degree in Nursing (BSN) or must obtain within three years of hire.
UMC Required: Bachelor’s degree in Nursing (BSN) or must obtain within three years of hire.
Experience Qualifications
Required: 2 years of professional nursing experience in specialty area.
Licenses and Certifications
- Louisiana State Board of Nursing - Registered Nurse License
- American Heart Association - Basic Life Support (BLS) Certification
