Job Summary and Responsibilities
As our Navigator RN, you will serve as a compassionate guide and advocate, empowering patients and their families to seamlessly navigate complex healthcare journeys.
- Proactively engage with patients and their families, conducting thorough assessments of their clinical, psychosocial, and practical needs, and identifying potential obstacles to their care plan progression.
- Meticulously coordinate appointments, facilitate communication between multidisciplinary care providers, educate patients on their diagnosis and treatment options, and connect them with vital community resources and support systems.
- Leverage robust clinical expertise and empathetic communication skills to build trust, alleviate anxieties, and empower patients to actively participate in their care decisions.
- Collaborate seamlessly with physicians, specialists, social workers, and other healthcare professionals, acting as a central point of contact and ensuring cohesive care delivery.
Job Requirements
- Bachelor's degree in Nursing
- Two (2) years of professional nursing experience
- Current licensure as a Registered Nurse issued by the Washington State Board of Nursing
- Current Healthcare Provider BLS Certification
- Strong problem-solving skills, particularly with respect to addressing barriers to care and navigating patients through their care progression
- Exceptional interpersonal skills
Preferred
- Master's degree in Nursing or related field
