About the Role
Responsible for delivering patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Directs and coordinates all nursing care for patients in the medical-surgical/Tele services based on established clinical nursing practice standards. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and achievement of desired patient outcomes. Serves as the Resource nurse when oriented and as directed.
Responsibilities
- Set priorities according to patient care needs.
- Complete initial assessments and re-assessments of patient care needs based on EMR review and/or direct observation.
- Document care according to diagnosis and established standards of practice; reassess as patient condition warrants and prepare transfer and discharge as appropriate.
- Recognize signs and symptoms of potential emergencies, including behavioral and physical changes, and initiate appropriate actions and timely interventions.
- Identify and properly report indicators of abuse.
- Practice National Patient Safety Goals (NPSG).
- Utilize appropriate methods to assess risk and prevent patient falls and skin breakdown; manage restraints.
- Perform and document legibly, timely, and accurate nursing interventions to meet patient care needs.
- Coordinate total nursing care using the plan of care to ensure continuity.
- Assume responsibility for teaching and documenting patient education consistently.
- Ensure complete follow-up instructions are given, with demonstrated or verbalized understanding by the patient prior to discharge, and document in the medical record.
Requirements
- Graduate of an accredited school of professional nursing.
- Current California RN license.
- BCLS and ACLS (AHA) certification, current and maintained.
- NIHSS certification, current and maintained.
- Proficient in oral and written communication skills.
- Ability to practice leadership, interpersonal skills, communication, teamwork, quality improvement, and professionalism.
Preferred Qualifications
- Chemotherapy proficiency.
- Proficiency in both written and oral English.
- Basic Dysrhythmia certificate.
