Provides direct patient care, in a medical/physician office setting, using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization.

Responsibilities

• Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources.
• Maintains accurate and timely documentation of patient care activities.
• Maintains effective communication with patients, families, and interdisciplinary team members.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Evaluates patient progress towards treatment goals and adjusts care plans accordingly.
• Evaluates patient response to medications and treatments.
• Evaluates patient’s physical, emotional, and psychological responses to care.
• Evaluates patient’s response to interventions and makes adjustments as necessary.
• Evaluates patient’s readiness for discharge and develops discharge plans.
• Evaluates patient’s need for follow-up care and arranges for referrals as necessary.
• Evaluates patient’s need for additional services and refers to appropriate specialists or community resources.
