RN MDS Coordinator

Responsibilities

• Conduct and coordinate the development and completion of the resident assessment
• Conduct resident assessments in accordance with current rules, regulations, and guidelines
• Implement resident CAA's
• Participate in facility survey readiness and throughout the survey
• Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident

Requirements

• Current unencumbered licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Vermont
• Two years of MDS and PPS experience
• Care Planning experience
