RN MDS Coordinator

About the role

The Grove of La Grange Park is a post-hospital rehabilitation and skilled nursing care facility known for its commitment to quality, compassion, and community integration. We are located in the suburban area of La Grange Park, offering a warm, personal touch in a professional setting.

Responsibilities

• Develop, monitor, audit, and modify each resident's care plan for their individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure residents are comfortable and safe through knowledge, skills, and attention to detail.

Requirements

• Unencumbered State of Illinois R.N. License.
• C.P.R. Certification.
• Minimum of one (1) year of experience as an R.N. in a Skilled Nursing Facility setting.
• Prior experience as a MDS Coordinator is preferable but not required.
• One (1) year of experience as a Certified Nurse Assistant (C.N.A.) is preferable but not required.

Qualifications

• Graduate of an accredited nursing program.
