Responsibilities
The MDS Care Plan Coordinator leads the MDS department, assists in discharge planning, and assesses short-term residents. Key responsibilities include maximizing reimbursement, ensuring Medicare compliance, managing short-term managed care cases, gathering information, establishing goals, providing interventions, coordinating interdisciplinary assessments, implementing care plans, monitoring care plan adherence, revising plans as needed, conducting weekly meetings, and completing accurate MDS forms.
Requirements
Candidates without prior MDS experience will not be considered. Must have a current state RN license and be in good standing. Must meet all applicable federal and state licensure requirements.
Skills
Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices, procedures, laws, regulations, and guidelines governing long-term care.
