Responsibilities

• Conduct thorough MDS assessments to identify patient needs and develop individualized care plans
• Develop and implement treatment plans that promote patient independence and well-being
• Collaborate with interdisciplinary team members to ensure comprehensive care
• Monitor patient progress and adjust care plans as needed
• Ensure compliance with regulatory requirements and industry standards

Qualifications

• Current RN licensure in the state of New York
• Minimum 3 years of prior experience in a similar role
• Ability to work in a fast-paced environment with a focus on patient-centered care
