RN MDS Assessment Coordinator

About the role

The RN MDS Assessment Coordinator assists in gathering clinical documentation and completing non-comprehensive MDS assessments, ensuring essential information from the assessment period is integrated into the care planning process.

Responsibilities

• Complete and oversee MDS assessments as directed by Director of MDS
• Ensure facility adherence to all regulatory requirements and current standards of practice in the MDS care planning process
• Educate the interdisciplinary team on the accuracy of clinical documentation and its importance in care delivery
• Coordinate timely completion of MDS assessments with the interdisciplinary team within RAI Manual guidelines and specified time frames
• Assist in reviewing clinical record accuracy of MDS coding and completeness of documentation
• Coordinate the Case Management process
• Act as liaison to the interdisciplinary team to discuss essential findings of a resident’s condition from MDS assessments
• Assist in transmission of MDS assessments, obtain validation records, and review them for needed corrections
• Perform other duties as assigned or deemed necessary by the Director of MDS

Requirements

• Nursing Degree from an accredited college or university, or graduation from an approved RN program
• Experience in long-term care and one year of job-related experience in the MDS process
• Familiarity with various computer software
• Efficient, organized, detail-oriented, and flexible
• Excellent time management skills with the ability to consistently meet task deadlines
