RN Registered Nurse MDS Assessor

About the role

We are seeking a talented and motivated RN Registered Nurse MDS Assessor to join our team at Highfield Gardens Care Center. Located in Great Neck, New York, we are a sub-acute and long-term care facility.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Monitoring MDS and care planning documentation for all residents; ensuring documentation is present in the medical record to support MDS coding.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing Director Of Nursing of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.

Requirements

• Current licensure as a Registered Nurse in the State of New York.
• Prior experience as a Registered Nurse in a related role.
• Strong clinical skills and knowledge of long-term care principles.
• Excellent communication and interpersonal skills.
• Able to work effectively in a fast-paced environment.
