About the role

The SNF CM Manager oversees member care management and coordination for members in Skilled Nursing Facilities (SNF). This role involves leadership and oversight of assigned SNF staff, collaboration with department management, and supporting continuity of care and quality care delivery.

Responsibilities

  • Collaborate with the Director of Care Management to ensure departmental operations effectively support continuity and delivery of quality patient care.
  • Provide leadership and oversight to assigned staff, ensuring adequate resources are available.
  • Monitor employee performance, identify opportunities for improvement or professional development, and facilitate coaching, counseling, and training.
  • Ensure timely, thorough, and accurate completion of employee performance evaluations in accordance with company policy.
  • Monitor SNF Care Management metrics, such as SNF Length of Stay, SNF Emergency Department (ED) Utilization, and SNF to Hospital Admissions.
  • Assess departmental performance on identified metrics and present findings to management.
  • Develop strategies using SNF Care Management metric data to achieve established departmental goals.
  • Provide ongoing training to staff on departmental workflows, policies, procedures, and updates.
  • Coordinate and lead departmental meetings.
  • Serve as a point of contact to resolve escalating concerns related to staff, SNF facilities, or patient care.
  • Establish and maintain professional relationships with contracted SNF facilities, providers, and internal staff to promote seamless patient transitions and maintain SNF length of stay below 13 days.
  • Ensure staff regularly review and evaluate each patient's plan of care to promote timely rehab progression.
  • Recommend process improvements using departmental performance data and staff feedback.
  • Ensure DOHC Contracted SNFs adhere to DOHC Contracts, including Rehabilitation and Nursing services.
  • Review and complete Specialty Referrals within the Traxit System for SNF/Custodial cases in a timely manner to prevent care delays.
  • Attend Daily Hospital reports to identify inappropriate/appropriate ER/Acute-to-SNF Transfers and assist accordingly.
  • Identify clinically complex and/or complex psycho-social SNF cases and assist with timely Hospice and/or Placement Transitions, including utilization prevention strategies.
  • Perform other duties as assigned.
  • Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care by positively shaping the customer experience through active listening, clear communication, and follow-through.

Qualifications

  • Completion of an accredited nursing program.
  • Bachelor's degree in Nursing (BSN) preferred.
  • Current California Registered Nurse (RN) License.
  • 3 years of recent acute care nursing experience.
  • Awareness of level of care, intensity of services, severity of illness, discharge screening, Skilled Nursing, and Home Health/Hospice criteria.
  • Ability to effectively communicate with individuals at all levels of the organization, patients, patient family members, providers, vendors, and others.
  • Detail-oriented with strong time management, multitasking, and prioritization skills.
  • Ability to utilize experience, judgment, and critical thinking to problem-solve and manage staff.
  • Proficiency in Microsoft Office applications (Word, Excel, Outlook, PowerPoint) and other computer software/applications.
  • Current California driver's license and proof of auto insurance.
