About the role
RN/LVN MDS Coordinator responsible for leading resident assessments and care planning in a skilled nursing and rehabilitation setting.
Key Responsibilities
- Complete and audit MDS 3.0 assessments, ensuring accurate coding and documentation.
- Support optimal Medicare/Medicaid reimbursement through compliance and accurate assessments.
- Collaborate with the interdisciplinary team to develop person-centered care plans.
- Monitor resident outcomes and contribute to quality improvement initiatives.
Qualifications & Skills
- Strong clinical judgment and attention to detail.
- Passion for compassionate, resident-focused care within a faith-based environment.
- Experience with MDS assessments and regulatory standards preferred.
