MDS Coordinator - LVN/RN

Stonegate Nursing & Rehab is seeking an experienced MDS/PPS Coordinator to join the team. The role requires an LVN or RN with extensive MDS/PPS and PSSR experience, as well as strong knowledge of long‑term care Medicaid, MDS, and Point Click Care.

Responsibilities

• Coordinate resident assessments, overseeing admission, annual, quarterly, Medicare, and change‑of‑condition assessments while ensuring timely communication with nursing and other departments.
• Ensure regulatory compliance by following all Federal and State guidelines for comprehensive MDS assessments.
• Monitor documentation by reviewing MDS and care‑plan files for accuracy, thoroughness, and organization.
• Utilize MDS software, maintaining expertise to ensure correct data entry and reporting.
• Lead interdisciplinary meetings, attending and coordinating Medicaid, ITTM, and care‑plan meetings for smooth team communication.
• Update care plans, adjusting and revising them as needed based on current assessments and resident status.
• Provide quality reporting by generating and distributing Quality Indicators reports to leadership to support care improvement.
• Submit MDS data, ensuring timely and accurate transmission of assessments per reporting standards.
• Stay current by attending professional development sessions to keep up with industry standards and best practices.
• Oversee ancillary data, collecting and reporting ancillary services to corporate billing for accurate processing.

Qualifications

• Minimum 2 years of long‑term care experience.
• Experience with MDS/TILE and fluency in MDS 3.0.
• Must have MDS/PPS and PSSR experience.
• Extensive knowledge of long‑term care Medicaid, MDS, and Point Click Care.
• Current, unencumbered, active license to practice as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in the state.
