RN/LPN/Therapist

Responsibilities

• Following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
• Understands OASIS and CMS requirements for home care.
• Provides direct patient care in the patient’s home.
• Implements the plan of care initiated by the RN, PT, or OT.
• Provides accurate and timely documentation consistent with the plan of care.
• Accurately assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Assists all clinicians or physicians in performing specialized procedures and duties, including preparing equipment and materials for treatments, observing aseptic techniques as required, and assisting the patient in learning appropriate self-care techniques.
• Participates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Counsels the patient and family in meeting nursing/therapy, and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements them prior to discharge of the patient, communicates D/C or ROC plan with the clinical manager.

Requirements

• Must have current licensure as an RN, LPN, or therapist in the state(s) of Butler, Clermont, Hamilton, and Warren counties.
• Must be able to work flexible hours, including evenings and weekends.
• Must have reliable transportation and valid driver’s license.
• Must be able to travel to various locations within the service area.

Qualifications

• Bachelor's degree in Nursing, Allied Health, or related field preferred.
• Current licensure as an RN, LPN, or therapist in the state(s) of Butler, Clermont, Hamilton, and Warren counties.
• Experience in home health care or related field preferred.

Skills

• Strong communication skills.
• Ability to work independently and as part of a team.
• Knowledge of OASIS and CMS requirements for home care.
• Proficiency in Microsoft Office Suite.

• Compensation is commensurate with experience.
