About the role
We are seeking a RN/LPN MDS Coordinator to join our dynamic team in Plainfield. The ideal candidate will be responsible for managing the MDS process, ensuring accurate documentation, and collaborating with interdisciplinary teams to provide comprehensive care plans.
Responsibilities
- Develop and implement MDS assessments and care plans
- Collaborate with interdisciplinary teams to ensure continuity of care
- Ensure accurate and timely documentation of MDS assessments
- Monitor and update resident profiles in the MDS system
- Provide education and training to staff on MDS processes and care planning
- Contribute to the development and implementation of MDS policies and procedures
- Assist with the preparation and submission of MDS reports to regulatory bodies
Requirements
- RAC-CT Certification Preferred
- Unencumbered RN or LPN/LVN licensure issued by the State Board of Nursing
- Valid CPR certification and active throughout employment
- Knowledgeable of nursing and medical practices and procedures, as well as state and federal regulations specific to nursing home operation and licensure
- Independent decision-making ability when necessary
- Leadership and supervisory skills
- Excellent follow-through and organizational abilities
- Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures
- Excellent analytical, written, and verbal communication skills
- Ability to lift 60-70lbs frequently, stand and/or walk throughout the scheduled shift, and comply with attendance policy and established nursing staff schedules
- Knowledge of computer systems, systems applications, and other office equipment
- Knowledge of local health regulations and ability to pass pre/post-employment physical exams, including drug screening, criminal background investigation, and reference inquiry
- Ability to promote positive interpersonal relationships and communicate verbally in a positive and professional manner
- Ability to relate positively and favorably to residents, families, co-workers, and work cooperatively with others
- Ability to attend in-service/education programs as required
- Agreement not to disclose resident protected health information and report suspected or known violations of such to the Administrator

Pay
The wage scale is determined by years of experience. Benefits & bonuses are available for full-time employees working over 30 hours per week.
