Community Health Partnership of Illinois is a non-profit organization that provides primary health care to migrant farmworkers and other medically underserved populations throughout northern and central Illinois. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.

Responsibilities

• Assessing and monitoring patients' health status
• Developing and implementing individualized nursing care plans
• Administering medications and treatments as prescribed
• Monitoring and interpreting vital signs
• Collaborating with other healthcare professionals to provide comprehensive care
• Educating patients and their families on medical conditions and treatments
• Maintaining accurate and up-to-date medical records
• Adhering to all hospital and regulatory policies and procedures

Qualifications

• A current RN license
• A Bachelor's degree in nursing or related field
• Previous experience in a clinical setting (preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassion and empathy for patients in need
