RN / LPN Intake

Key Responsibilities

• Receive client referrals and assess their appropriateness for admission to Atrio Home Health services.
• Verify the need for skilled care and confirm insurance coverage.
• Accurately enter data into Atrio’s systems.
• Collaborate with referral sources to ensure clients’ care needs are met during their initial home visit with Atrio staff.
• Monitor the status of current clients who have been transferred to hospitals or facilities, completing necessary documentation.
• Discuss Atrio Home Health’s philosophy, policies, procedures, services, and programs, while educating clients about home health care as needed.
• Communicate with physicians, discharge planners, and other personnel to ensure continuity of care and a safe transition plan.
• Respond promptly to potential referrals, adjusting schedules as necessary to maintain a high level of customer service.
• Actively work to increase referrals to Atrio Home Health.

Qualifications and Requirements

• Current and valid Michigan RN or LPN license.
• CPR certification required.
• 1 year minimum of home health experience.
• Exceptional attention to detail and ability to produce high quality documentation.
• Strong computer skills, specifically in Excel and Microsoft.
