Responsibilities

• Provide direct patient care in a variety of settings, including skilled nursing facilities, home health care, and assisted living communities
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
• Conduct initial and ongoing assessments to identify patient needs and develop effective care strategies
• Administer medications, treatments, and therapies as ordered by physicians
• Collaborate with healthcare teams to coordinate patient care and services
• Maintain accurate and detailed records of patient care and treatment
• Participate in ongoing professional development and continuing education to stay current with best practices and industry advancements

Requirements

• Current licensure as an RN or LPN in the State of Massachusetts
• 0 years of experience as an RN or LPN (new graduates welcome to apply!)
• Ability to work effectively in a fast-paced, dynamic environment
• Strong communication and interpersonal skills
• Compassionate and empathetic approach to patient care
• Ability to work varied shifts, including days, evenings, and weekends
