About the Role

The Registered Nurse (RN) in Long Term Care delivers comprehensive nursing care to residents, addressing both chronic health management and daily living needs. This role involves conducting ongoing assessments, monitoring resident conditions, and coordinating individualized care plans to promote comfort, dignity, and quality of life. The RN collaborates with physicians, families, and interdisciplinary team members to ensure timely communication, safe care transitions, and adherence to regulatory and organizational standards. This position provides clinical leadership and oversight in a team-oriented environment, fostering a stable, resident-centered setting through compassionate and consistent care.

Responsibilities

  • Perform detailed assessments and monitor resident conditions, reporting significant changes to providers and communicating updates to families as appropriate.
  • Develop, implement, and revise care plans to address resident needs, ensuring interventions support safety, comfort, and functional abilities.
  • Provide and oversee direct nursing care, including medication administration, treatments, and coordination of supportive services.
  • Supervise and support LPNs, CNAs, and other care staff to ensure consistent delivery of quality resident care and adherence to facility policies.
  • Document assessments, interventions, and communications accurately in the electronic health record to support regulatory compliance and continuity of care.
  • Participate in quality-improvement efforts, safety initiatives, and interdisciplinary meetings to enhance resident well-being and operational effectiveness.
