RN - Liaison Weekends

Requirements

• Current unrestricted AR RN license
• BLS certification
• Strong assessment skills with critical thinking and high level of problem solving within a healthcare facility
• Strong documentation and communication skills
• Able to stay calm and cope with human suffering/emergencies
• Prefer hospice experience, home health, or acute care nursing experience with computer charting experience
• Use of personal vehicle, valid driver's license, and current auto insurance

General Description

• Integral part of the Interdisciplinary Team
• Works closely with the Medical Director
• Primarily responsible for referral, coordination, and execution of the admission process into Hospice care
• Nurse needs to effectively communicate the Hospice concept to families and patients
• Develops the plan of care and communicates patient/family care needs to the team
• Trains and educates patient/caregivers and establishes appropriate frequency of disciplines to meet needs
• Deals with human suffering while maintaining professional boundaries
• Must be flexible and change schedule at a moment's notice to meet patient needs
• Required to travel daily and manage own schedule effectively
• Must have excellent clinical skills and be able to identify problems, perform head to toe assessment, educate, consult with the physician, and execute orders to meet patient's needs
• Assists with admissions as needed
