About the role

The Registered Nurse (RN) utilizes the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide professional nursing services to patients/residents within the hospice setting. This includes documenting human responses to actual or potential health problems in the home health environment, collaborating with patients/residents, families, and inter-professional colleagues (such as physicians) to plan, implement, and evaluate care. The role involves competency in practicing within the full scope of nursing expertise/knowledge, adhering to age and population-specific standards, and ensuring continuity of care through assessment, education, triage, and various nursing interventions. The RN functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.

Responsibilities

  • Provide comfort-focused, patient-centered care to individuals and families during end-of-life stages.
  • Support symptom management, emotional comfort, and dignity for every patient.
  • Collaborate with an interdisciplinary team to plan, implement, and evaluate care.
  • Implement the nursing process, including coordination and continuum of care, patient assessment, education, triage, and other nursing interventions.
  • Adhere to the Scope and Standards of Practice and the Code of Ethics for Nurses as published by the American Nurses Association.

Requirements

  • Graduate from a nationally accredited nursing program preferred (e.g., CCNE, ACEN, NLN CNEA).
  • Bachelor's degree in nursing preferred.
  • Minimum of one year of professional nursing experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice occurs and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and maintains required department-specific competencies and certifications.
  • Basic Life Support (BLS) certification required.
  • Certificate for Hospice and Palliative Nurse (CHPN) preferred.
  • Depending on location, a valid driver’s license may be required.

Pay

$33 - $48 per hour.

Schedule

PRN (as needed) with varied shifts and weekly hours.
