RN - Hospice PRN

About the role

Hospice RN providing end-of-life skilled nursing care in patients' homes and facilities.

Responsibilities

• Assess patient needs and develop and coordinate individualized care plans with the healthcare team.
• Perform tasks such as vitals, wound care, IV, ostomy care, blood draws, and injections.
• Collaborate with physicians and multidisciplinary teams, documenting visits and care plans accurately.
• Manage discharge planning, provide emotional and spiritual support, and respond to patient and family calls.
• Ensure compliance with policies, procedures, and regulations of home and hospice care.

Qualifications

• Graduate of an accredited nursing program or equivalent.
• Valid RN license in applicable states; BLS certification.
• Strong communication skills, alertness, and ability to work independently.
• Physical ability to perform patient care tasks, including lifting and mobility assistance.
• Valid driver’s license, insurance, and reliable transportation.

• Ensure safety, accurate documentation, and effective communication with patients, families, and healthcare team.
