About the role
Hospice RN providing end-of-life skilled nursing care in patients' homes and facilities.
Responsibilities
- Assess patient needs and develop and coordinate individualized care plans with the healthcare team.
- Perform tasks such as vitals, wound care, IV, ostomy care, blood draws, and injections.
- Collaborate with physicians and multidisciplinary teams, documenting visits and care plans accurately.
- Manage discharge planning, provide emotional and spiritual support, and respond to patient and family calls.
- Ensure compliance with policies, procedures, and regulations of home and hospice care.
Qualifications
- Graduate of an accredited nursing program or equivalent.
- Valid RN license in applicable states; BLS certification.
- Strong communication skills, alertness, and ability to work independently.
- Physical ability to perform patient care tasks, including lifting and mobility assistance.
- Valid driver’s license, insurance, and reliable transportation.
