RN Hospice House- Ray Hickey Hospice House

PeaceHealth is seeking a Per Diem/Relief RN for the Ray Hickey Hospice House. This position is represented by a collective bargaining agreement.

About the Role

This position is responsible for providing direct skilled nursing care and coordination/management of patients admitted to hospice house inpatient services. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention, and clinical/goal outcome evaluation.

Responsibilities

• Perform initial evaluation of patient and apply admission criteria to determine if patient is a safe candidate for home health/hospice services.
• Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the hospice plan of treatment.
• Perform physical assessments, develop, coordinate, modify, and manage written individualized plans of care as ordered by the primary physician to meet patient’s long and short-term medical goals and appropriate/timely hospice outcomes.
• Develop and clinically manage the Home Health Aide’s care plan according to the patient’s evolving personal care needs.
• Coordinate care, collaborate, and communicate with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
• Perform discharge planning appropriately toward patient-specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
• Provide accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and Agency guidelines.
• Assist with the plan and implementation of family Bereavement services following patient death.
• Perform other duties as assigned.

Requirements

• Accredited School of Nursing (Required)
• Nursing Bachelor's Degree (Preferred: Bachelor of Science in Nursing)
• Minimum of 1 year of home health or hospice/end-of-life nursing experience (Required)
• Hospice experience (Preferred)
• Demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient’s needs (Required)
• Registered Nurse license in the state of practice (Required upon hire)
• Basic Life Support certification (Required upon hire)
• Food Handlers Card - Washington (Required upon hire)

• Basic computer skills including MS Office applications (Required)
• Well organized, flexible, and able to function independently (Required)
• Cooperative working skills, leadership skills, and interpersonal skills with the ability to work effectively with a wide variety of patients, families, and staff (Required)

Working Conditions

• Lifting/Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
• Fine motor skills to grasp and control medical equipment and perform precise procedures.
• Push/Pull: Frequently up to 45 lbs. force (e.g., WOW, medical carts).
• Perform hands-on CPR for at least 20 minutes using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA). Frequency should be seldom or occasionally.
• Ability to move around the area with frequent sitting.
• Bending, stooping, squatting, reaching, and kneeling frequently.
• Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.
• Exposure to biohazard, body fluids, and airborne particles.
• Must be able to complete tasks in a noisy environment.
• Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
• Ability to communicate and exchange accurate information.

Pay

The salary range for this job opening is $54.65 – $82.66 per hour. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, and seniority.
