About the role
Pride Health is hiring a Hospice Home Health RN for one of our clients located in PA. The position is a contract role scheduled Monday through Friday from 8 AM to 4:30 PM, with two nights of on-call duty each week (Monday through Thursday).
Responsibilities
- Coordinate end-of-life care for terminally ill patients, serving as the primary liaison between families, physicians, and the hospice care team.
- Assess patient needs, manage pain and symptoms, and oversee the execution of personalized care plans to ensure comfort and dignity.
- Manage a designated patient caseload, conducting initial and ongoing physical, psychosocial, and spiritual assessments of patients in their homes or residential facilities.
- Educate caregivers on patient care, disease progression, and the grieving process.
- Maintain accurate, compliant electronic medical records (EMR) and update physicians on patient status changes.
- Participate in regular interdisciplinary treatment team (IDT) meetings to review care plans and adjust interventions as the patient's condition changes.
Requirements and Qualifications
- Active RN License
- BLS Certification
- Hospice experience

Pay
Pay Rate: $50-55/hour
Schedule
Schedule: Mon-Fri 8A-4:30pm, On Call: 2 nights on call, Mon-Thurs
Productivity Expectations
Coverage Areas: CAMBRIA AND SOMERSET COUNTIES
Productivity expectations: 5-6 PATIENTS/DAY
