RN Hospice Baylor Schedule

About the role

VNA Health Group is a hospice care provider dedicated to delivering exceptional care in patients' homes. We are seeking a skilled Registered Nurse (RN) to join our team.

Responsibilities

• Deliver skilled nursing services and coordinate care for patients in their homes.
• Conduct comprehensive assessments and create individualized care plans.
• Maintain accurate and timely documentation using our EMR system.
• Educate and empower patients and families in understanding treatment plans and self-care strategies.
• Collaborate with physicians, case managers, and interdisciplinary team members to ensure coordinated care.

Qualifications

• Current NJ RN license required.
• Minimum 1 year of clinical nursing experience; home care experience preferred.
• Strong communication, clinical judgment, and critical thinking skills.
• Proficient with EMR and digital tools.
• Reliable car, valid driver’s license, and proof of auto insurance required.

Pay

The compensation range for this position is USD $43.00 to USD $51.00 per hour.

Schedule

This Baylor position works every Saturday and Sunday, 12-hour shifts, and is paid for 30 hours.
