About the role
Under the supervision of the Clinical Manager, the RN field clinician is accountable for assessing patients for eligibility for homecare services, admitting eligible patients following the admission process, and managing patients from admission through transition into the community.
Responsibilities
- Perform admissions, revisits, resumptions of care, recertifications, and discharges to ensure safe and effective patient care
- Coordinate interdisciplinary care and monitor and adjust treatment to achieve positive therapeutic outcomes
- Identify opportunities for process improvement
- Ensure responsibilities are carried out in accordance with organization policies and procedures, as well as Federal, State, and accreditation regulations
Schedule
The Baylor RN is required to work two (2) 11.5-hour shifts on Friday and Saturday, Saturday and Sunday, or Sunday and Monday. They work 23 hours and are paid for 34.5 hours.
Requirements
- Equivalent education required for RI Registered Nurse licensure required; Bachelor’s Degree in relevant discipline strongly preferred
- A minimum of one (1) year progressive clinical experience required
- Previous experience in an acute care or home health care environment setting preferred
- Valid RI Registered Nurse license required
- Valid CPR certification required
- Valid driver’s license and automobile liability insurance are required
