About the role

This position is under the supervision of the Clinical Manager and is responsible for assessing patients for homecare services, managing their care from admission through discharge, coordinating interdisciplinary care, and ensuring safe and effective patient care.

Responsibilities

  • Assessing patients for eligibility for homecare services
  • Admitting eligible patients following the admission process
  • Managing patients from admission through transition into the community
  • Carrying out admissions, revisits, resumptions of care, recertifications, and discharges
  • Cooking interdisciplinary care
  • Monitoring and adjusting treatment to achieve positive therapeutic outcomes
  • Identifying opportunities for process improvement

Requirements

  • Equivalent education required for RN licensure
  • Bachelor’s Degree in relevant discipline preferred
  • One (1) year progressive clinical experience required
  • Previous experience in an acute care or Home health care environment setting preferred
  • Valid RN license required
  • Valid CPR certification required
  • Valid driver’s license and automobile liability insurance are required

Qualifications

Equivalent education required for RN licensure; Bachelor’s Degree in relevant discipline strongly preferred. One (1) year progressive clinical experience required. Previous experience in an acute care or Home health care environment setting preferred. Valid RN license required. Valid CPR certification required. Valid driver’s license and automobile liability insurance are required.

Skills

Ability to work two (2) 11.5 shifts per week, working 23 hours and being paid for 34.5 hours. Must be able to perform the essential job-specific functions either unaided or with the assistance of a reasonable accommodation.
