RN Hospice

About the Role

Join a hospice team as a Registered Nurse (RN) providing compassionate end-of-life care in a 16-patient hospice house. The position involves evaluating patients, developing and implementing personalized care plans, and coordinating care across an interprofessional team. The RN assesses patient needs while respecting emotional, spiritual, cultural, and individual preferences, and promotes patient-centered, evidence-based practices to ensure quality outcomes.

Responsibilities

• Assess patient needs and develop individualized care plans
• Implement and evaluate nursing care, ensuring safety and quality
• Monitor patient condition changes and communicate updates to families and the team
• Advocate for patient dignity and comfort
• Communicate care plans and updates with patients, families, and team members
• Promote compassionate, culturally sensitive care, respecting patient preferences
• Document interventions and patient progress accurately in electronic medical records

Requirements

• Valid RN license and Driver's License
• Preferred hospice experience
• Strong assessment, communication, and critical thinking skills
• Ability to collaborate effectively with diverse populations
• Experience with care coordination and documentation
