About the role
Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager coordinates and delivers compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing individualized care plans, and collaborating with an interdisciplinary team to ensure high-quality hospice care.
Responsibilities
- Conduct comprehensive assessments and develop individualized care plans based on patient needs and goals, ensuring optimal comfort and symptom management.
- Administer medications, monitor symptoms, and adjust treatment plans to provide optimal relief.
- Work closely with physicians, social workers, chaplains, and other healthcare professionals to coordinate ongoing holistic care.
- Provide support, resources, and education on disease progression, hospice philosophy, and available support services to patients and families.
- Support patients and families in making informed decisions about care and treatment options.
- Provide compassionate care addressing emotional, spiritual, and psychological needs.
- Maintain accurate, compliant, and timely records of patient care, interventions, and progress while adhering to hospice regulations.
- Travel to patients' homes, nursing facilities, and hospitals to provide direct care.
- Continuously seek opportunities for learning and skill enhancement in hospice and palliative care.
- Actively participate in safety initiatives and risk-mitigating measures; complete all position and unit safety-related competencies and requirements on time.
- Perform other duties as assigned.
Requirements
- Associate’s degree in nursing (Required)
- 2 years of relevant experience (Required)
- Michigan Registered Nurse (RN) license upon hire (Required)
- State of Michigan Driver’s license upon hire (Required)
- Bachelor’s degree (Preferred)
- 1 year of relevant experience (Preferred)
