About the Role

Join Logan Health Hospice as an independent, self-directed RN and return to the heart of nursing—providing compassionate, person-centered care at the bedside. You’ll build meaningful relationships with patients and families, guiding them through symptom management, education, and comfort care during end-of-life journeys. This role offers a change of scenery from clinic or inpatient settings, allowing you to work in patients’ homes throughout the Flathead Valley.

Responsibilities

  • Manage your own patient caseload, traveling to patients’ homes for visits.
  • Serve as the primary nursing contact for patients, coordinating care from admission through end of life.
  • Conduct in-home assessments, symptom management, and comfort-focused interventions.
  • Educate patients, families, and caregivers on care expectations and symptom management techniques.
  • Collaborate with the interdisciplinary team—physician, social workers, chaplains, CNAs, LPNs, and volunteers—to create customized care plans.
  • Attend regularly scheduled Interdisciplinary Team (IDT) meetings.
  • Provide emotional support and guidance to families navigating anticipatory grief.
  • Participate in on-call shift rotation and perform ad-hoc patient visits as needed.
  • Document visits and coordinate medications, equipment, and supplies to ensure seamless care.
  • Develop, implement, and coordinate individualized, goal-directed nursing care using the nursing process.
  • Maintain nursing standards of care, adapting to changing workloads and patient conditions.
  • Transcribe and implement provider orders and treatments accurately and efficiently.
  • Problem-solve issues affecting patient care and report changes in patients’ physical or mental conditions.
  • Follow infection control procedures and provide patient education per department protocols.
  • Assess and anticipate patient needs, coordinating interventions as defined by policy and provider orders.

Requirements

  • Current Montana RN license or a multi-state compact license with authorization to practice in Montana.
  • BSN degree preferred.
  • Minimum of two (2) years’ experience in a medical setting (e.g., physician office, clinic, or hospital); hospice or related experience preferred.
  • Current Driver’s License required.
  • Clean driving record for the previous thirty-six (36) months (no major offenses such as DUI, felony convictions involving a motor vehicle, suspended license, or three or more moving violations).
  • Personal vehicle with state law minimum liability coverage for business use.
  • Knowledge of professional nursing theory, medical terminology, and principles of professional nursing practice.
  • RQI Provider (BLS) certification required within 7 days of hire or before independent patient care.
  • Understanding of HIPAA and confidentiality laws governing provider-patient relationships.
  • Familiarity with medical office equipment, clinical laboratory methods, and universal precautions.
  • Knowledge of OSHA regulations, manage care carrier restrictions, and protocols for storing hazardous materials.

Schedule

Full-time, 40 hours per week, 8-hour day shifts. On-call and holiday rotation required.
