Responsibilities
- Facilitates Hospice and Palliative Care referrals from all sources and obtains reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program.
- Provides palliative care & hospice services based upon patient assessment, including direct patient care.
- Utilizes nursing assessment skills to determine presence of symptoms and response to interventions for those symptoms.
- Assesses patient needs across multiple domains (physical, psychological/social, behavioral, functional, financial, resource, spiritual).
- Serves as a consultant to discuss goals of care with the patient/family and communicates those goals to the appropriate care team.
- Reviews referrals and delegates to the appropriate hospice/palliative care staff based on patient needs and staff availability.
- Performs Hospice consult reviews and coordinates a clinical assessment, determining if patient is appropriate for hospice, and communicates with the patient/family about hospice philosophy, criteria, and site of care.
- Assumes role of hospice RN with hospital hospice patient, developing and implementing a hospice plan of care and subsequent rounding, if necessary.
- Acts as a clinical resource for all members of the hospice /palliative team.
- Maintains a working knowledge of all services available within the Home and Transitions Service Line.
- Completes comprehensive and accurate documentation that clearly reflects interventions performed.
- Performs as a member of an interdisciplinary team.
- Affords assistance with orientation and mentoring of new team members as appropriate.
- Serves as a resource person & professional role model.
- Associate Degree in Nursing
- Nursing License from State of New Mexico or Compact State
- BLS certification REQUIRED at time of hire
- ACLS certification
- PALS certification
- One year of hospice experience required
- One year of acute care experience strongly preferred
- Current drivers license and auto insurance
- Basic typing competency
