Hospice Clinical Liaison RN

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Work with the Hospice Care Consultants to develop and implement individualized marketing plans.
• Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
• Act as a liaison with AccentCare Hospice Medical Director, Team Physicians, and Hospice Physicians on behalf of providers and patients/families.
• Make inpatient visits at contracted facilities and communicate with Admissions Department, Team Directors, and/or Primary Nurse.
• Provide consultation on hospice and palliative care.

Requirements

• One (1) year of RN experience in the local market with strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques.
• Previous medical sales experience preferred.
• Licensed RN currently licensed as a registered nurse in the state of agency operation.
• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order.

Schedule

Monday through Friday with limited on-call.

Location: Atlanta, GA and some of Gwinnett County.
