Hospice Clinical Liaison Nurse

Role: Hospice Clinical Liaison RN Responsibilities Build and maintain relationships with hospitals, physicians, and community partners to facilitate seamless transitions to hospice care.Assess clinical appropriateness for hospice services and educate patients and families on end-of-life care options.Conduct timely bedside evaluations to determine patient needs.Collaborate with the hospice interdisciplinary team to develop and implement care plans.Ensure accurate documentation and smooth admission processes for hospice patients. Qualifications And Skills Registered Nurse (RN) licensure.Compassionate, patient-centered approach to care.Strong communication and relationship-building skills.Ability to assess clinical needs and educate families effectively.Experience in end-of-life or hospice care preferred.
