About the role
This position is for a Registered Nurse (RN) providing in-home patient care, assessing, planning, and implementing individualized care plans.
Responsibilities
- Conduct patient assessments
- Educate patients and families on health conditions, procedures, and medications
- Manage complex needs such as wound care, tracheostomies, G-tubes, catheters, ventilators, and medication administration
- Deliver personalized patient care
- Build long-term relationships
- Support patient independence and health milestones
