RN Home Visitor -- Family Connects New Orleans -- FT

About the role

We provide home visiting to all families with newborns in a specified geographical area. Our team includes clinical and non-clinical staff who work together to support families in 12 domains of physical and psychosocial wellbeing.

Responsibilities

• Supports and assesses family needs in 12 domains of physical and psychosocial wellbeing and facilitates referrals and connections to community agencies.
• Schedules, visits, and supports families in the community using the evidence-based home visiting protocol.
• Works with the team to ensure appropriate referrals and connections to community resources and assists parents in making and attending appointments.
• Maintains eight open visit slots, conducts and documents at least six home visits per week, responds to client needs through flexible scheduling, and documents and sends letters within 48 working hours of the visit.
• Attends weekly team, clinical supervision, staff, and other meetings and supports and encourages development of staff and programs.

Requirements

• Associate's degree in nursing.
• Bachelor’s degree in nursing preferred.
• 1 year of maternal/child and home visiting experience required.
• Experience in lactation education, domestic violence, substance abuse/depression, social work, or related field preferred.
• Registered nurse (RN) license in state of practice required.
• BLS certification from American Heart Association required.
• Valid driver’s license in state of practice required.
• Preferred: Certified Lactation Educator or Consultant.
• Fluency in Spanish preferred.

• Proficiency in using computers, software, and web-based applications.
• Effective verbal and written communication skills and ability to present information clearly and professionally.
• Interpersonal skills and ability to work in a team of clinical and non-clinical staff.
• Effective time-management, organization, and multitasking skills.

• Ability to work with the nurse supervisor to implement practice improvement and clinical development plans.
• Ability to work in a team of clinical and non-clinical staff.

• Ability to act as a liaison between families and other community agencies and serve as a family advocate.

Pay

The pay range for this position is $XX.XX - $XXX.XX per hour.

Schedule

The schedule for this position is Monday through Friday, with some flexibility in scheduling home visits.
