About the Role

The Registered Nurse Home Visits - Family Connects provides family-centered, evidence-based home visiting services to families with newborns in Chicago.

Responsibilities

• Conduct home visits or virtual appointments, documenting observations and assessments in real time.
• Follow established protocols for family assessments, including postpartum depression screening.
• Assist in coordinating community resource referrals and support services.
• Participate in team meetings, supervision, and community-based teaching sessions.
• Maintain scheduling flexibility to accommodate family needs, including evenings and weekends.
• Assess family needs across physical and psychological domains, support new parents, and conduct physical assessments of mothers and infants.
• Promote a safe environment for newborns.

Requirements

• Bachelor's degree in nursing with current RN license or equivalent.
• At least one year of relevant experience.
• CPR certification, valid driver’s license, and reliable transportation.
• Proficiency with computers, including Word, Outlook, and Excel; comfortable using laptops in-home.
• Bilingual skills in Spanish and English preferred, with ability to utilize translation services.
• Experience in clinical settings, child/maternal health, social work, or related fields is advantageous.
