RN Home Health - Weekends

About the Role

Provide direct patient care in home settings, coordinate care plans, and ensure compliance with regulations.

Responsibilities

• Develop, implement, and update patient care plans in collaboration with physicians and interdisciplinary teams.
• Perform comprehensive nursing assessments, including OASIS and medication reconciliation.
• Coordinate and supervise clinical care, including tasks assigned to home health aides and LPN/LVNs, ensuring regulatory compliance.
• Accurately document patient care and communicate effectively with physicians, families, and staff.
• Participate in staff orientation, performance improvement, patient safety initiatives, and efforts to reduce hospitalizations.
• Maintain professionalism, punctuality, ongoing learning, and participate in on-call/weekend rotations.

Requirements

• Current RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Preferred Qualifications

• CPR certification.
• 1+ years of clinical experience.

Pay

Salary ranges from $78,288 to $117,432 annually, based on experience and qualifications.
